Most people when they're ill want to remain at home with their loved ones.
Indeed, the majority of our patients are cared for in the community rather than at the Hospice in Selly Park.
Our Community Palliative Care Team, made up of specialist nurses also called clinical nurse specialists, works closely with general practitioners, community nurses and other health and social care workers.
Our nurses have extensive experience and knowledge of palliative care.
They work with and advise GPs and community nurses to make sure patients have effective symptom relief.
They can also provide psychological support to patients and families referred to them.
Furthermore t he Team acts as a link between the community and the Hospice's other services.
The specialist nurses liaise with the Inpatient Unit, Day Hospice, Family and Carer Support Team, physiotherapists, occupational therapists and complementary therapists so that patients can benefit from these services.
The team is available from Monday to Friday from 9 am to 5 pm and a member is on duty on Saturdays, Sundays and Bank Holidays for the same hours.
Outside these hours, one member is on-call for specialist palliative care advice which cannot wait until normal working hours.
This is not an emergency service and does not replace the service provided by GPs (or out-of-hours providers) or community nurses.
Sometimes, the reasons patients are referred to the Community Palliative Care Team are resolved.
For example, symptoms may no longer be a problem or there may be an overall improvement in their medical condition.
Patients may then be discharged from the team, hopefully when they are feeling better and able to get on with their lives.
People can be re-referred at anytime if they need the specialist care provided by the Team again.
